Frequently Asked Questions
General Questions
Stimulate Insulin Release: GLP-1 medications help the pancreas release insulin, lowering blood sugar levels.
Suppress Glucagon: They inhibit glucagon, a hormone that increases blood sugar, promoting better glucose control.
Slow Gastric Emptying: These drugs slow down digestion, leading to a gradual release of nutrients and reduced blood sugar spikes.
Enhance Satiety: By promoting a feeling of fullness, GLP-1 medications help reduce food intake and support weight loss.
These medications have become increasingly popular in telehealth weight loss programs due to their dual benefits of improving metabolic health and promoting sustainable weight loss. When used under medical supervision, GLP-1 medications can be a powerful tool in managing obesity and type 2 diabetes, making them a cornerstone in modern telehealth weight loss strategies.
Typically, initial weight loss is around 5% of body weight, which can increase to up to 15% over a 12-week period. Healthy weight loss is generally 1-2 pounds per week, as rapid loss is not advisable from a medical standpoint.
For the best results, GLP-1 medications should be combined with a healthy lifestyle, including diet and exercise. It’s important to consult with a healthcare provider for personalized expectations and guidance tailored to your unique journey.

1. Sublingual Drops
Effectiveness: Drops administered under the tongue are often considered the most effective sublingual form for semaglutide & tirzepatide. This method allows the medication to be absorbed directly through the mucosal membranes, offering faster and more complete absorption.
Advantages: Drops are easy to dose, and absorption through mucosal tissue can be enhanced by specific compounding agents. This form generally bypasses first-pass metabolism (the breakdown of a drug in the liver), potentially making it more effective at lower doses.
2. Troches (Lozenges)
Effectiveness: Troches are also absorbed through the mucosal tissue under the tongue or inside the cheek, making them a good alternative to drops. They typically provide a slower release, as the lozenge dissolves over time, which could be beneficial for sustained release but may not offer as immediate absorption as drops.
Advantages: Troches are discreet and portable, and they can be flavored for a more pleasant experience. They still offer sublingual absorption but may have slightly delayed effectiveness compared to drops.
3. Capsules
Effectiveness: Capsules are usually the least effective option among the three, especially if swallowed, because they must go through the digestive system before absorption. Oral bioavailability for semaglutide & tirzepatide can be quite low, meaning that much of the medication may be lost in the digestive process, which can also lead to more gastrointestinal side effects.
Advantages: Capsules are easy to take and have a consistent dose. However, they are better suited for patients who prefer traditional oral forms or who have trouble with sublingual administration but may need a higher dose for effectiveness.
Conclusion:
Sublingual drops tend to be the most effective for fast, direct absorption, with troches being a close second due to their prolonged release under the tongue. Capsules, while convenient, may require higher doses to achieve similar effects due to lower bioavailability.
If you're considering offering semaglutide & tirzepatide in a telehealth setting, sublingual drops and troches may provide better patient outcomes and adherence, especially for those seeking faster and more reliable absorption without injections.
